Instructions for First Time Athletes at SBHS
FOLLOW INSTRUCTIONS FOR REGISTRATION AND HEALTH FORM COMPLETION
Each school year, all athletes and parents/guardians are required to complete the online sports registration and required health forms prior to participation.
Online registration and health forms are located here.
The School Code for Sportsware is: Wolves.
Detailed registration instructions and a video tutorial are located below.
Quick Directions:
Go to swol123.com and create an account (if you already have an account, please do not make another).
SCHOOL CODE IS: wolves
Wait for confirmation email from Sportsware (may take up to 24 hours).
Once you get this email, click the link to create a password and SIGN IN!!
Read & Sign the 3 forms in Sportsware (under the forms tab on the left)
OR, You can print them off via the documents below and bring them to Sierra in the Athletic Training office.
All forms are to be handed into our Athletic Trainer: Sierra Villemaire (svillemaire@sbschools.net)
Fill out the "Medical Info" and the "Athlete Info"
Upload a copy of the athletes most recent Physical/Well-Exam, signed by their doctor.
Go to the "Forms" tab, press the "Add" button
Athletes will NOT be allowed to participate in athletics at SBHS until registration and all health forms, including the Physical/Well-Exam are completed.
Paperwork review and clearance for participation may take up to 24 hours from the time of registration completion.
